The Immunization Gap Agent reconciles multi-source vaccine history, applies ACIP age- and risk-based rules, and drives scheduling plus targeted campaigns to close preventive immunization gaps
The agent consolidates immunization history, evaluates each patient against ACIP recommendations including catch-up and high-risk indications, suppresses invalid or contraindicated recommendations, and orchestrates outreach and appointment slots for due vaccines with documentation of administration or refusal.
Reconcile Immunization History
- Ingest EHR immunizations, IIS/state registry queries, pharmacy claims, and patient-reported vaccines
- Deduplicate doses by CVX/MVX, date, and lot where available
- Flag conflicts (e.g., mismatched product or impossible spacing) for clinical review
Apply ACIP Schedule and Gap Logic
- Evaluate age-based, seasonal, and risk-based recommendations (influenza, COVID-19, pneumococcal, shingles, HPV, Tdap, childhood series)
- Apply catch-up schedules, minimum intervals, and series completion rules
- Suppress recommendations for allergies, pregnancy timing, recent dose, or documented immunity
Drive Scheduling and Outreach Campaigns
- Segment campaigns by vaccine type, age band, and clinic capacity
- Send reminders via portal, SMS, and mail with self-scheduling or nurse clinic slots
- Coordinate standing-order nurse visits and pharmacy partnership pathways where allowed
Document Closure and Report Coverage
- Capture administered doses, patient refusals, and deferrals with reason codes
- Write back to EHR and initiate IIS submission where configured
- Publish coverage rates, gap closure velocity, and equity stratifications
