Health systems exchanging data across multiple EHRs face broken handoffs, mismatched patient identities, and incomplete clinical documents that force clinicians to re-collect history

Point-to-point interfaces are brittle, reconciliation rules live in tribal knowledge, and failed exchanges surface only after care delays

The EHR Interoperability Agent orchestrates standards-based exchange, applies identity and clinical reconciliation rules, and ensures complete, usable records reach the receiving system of care

The agent coordinates ADT, CCD/C-CDA, FHIR, and query-based document retrieval across EHR endpoints, resolves patient identity, reconciles meds/allergies/problems, validates payload completeness, and routes exceptions to interface and clinical informatics teams.

1

Orchestrate Exchange Triggers

  • Detect events requiring exchange (referral, transition of care, HIE query, scheduled visit)
  • Select transport and standard (Direct, XCA/XDS, FHIR API, HL7 v2) based on partner capability
  • Initiate document generation or query with correct purpose-of-use and consent checks
Outcome: Exchange jobs launched with partner-specific standards and consent validated.
2

Resolve Identity and Match Records

  • Apply enterprise MPI matching using demographics, identifiers, and probabilistic scores
  • Handle ambiguous matches with human-in-the-loop queues
  • Link external MRNs to internal patient IDs for write-back
Outcome: High-confidence patient match or queued exception for HIM review.
3

Reconcile Clinical Content

  • Normalize medications, allergies, problems, and immunizations against local terminologies
  • Apply reconciliation rules for duplicates, conflicts, and more-recent external data
  • Prepare clinician-facing reconciliation views for meds and allergies at transitions
Outcome: Reconciled clinical sections ready for EHR import or clinician confirmation.
4

Validate Delivery and Monitor SLAs

  • Confirm ACK/receipt, payload completeness, and successful EHR incorporation
  • Retry transient failures with backoff and escalate persistent errors
  • Report exchange volume, latency, match rates, and clinical reconciliation metrics
Outcome: Reliable cross-EHR exchange with operational visibility and exception handling.
